Researchers find MIS-C Associated with Myocardial Injury

▫️Using Sensitive Parameters to assess Cardiac function, researchers at Children's Hospital of Philadelphia (CHOP) have found that Cardiac involvement in Multisystem Inflammatory Syndrome in Children (MIS-C) differs from Kawasaki disease (KD) and is associated with Myocardial injury.
▫️The findings were published recently in the
Journal of the American College of Cardiology.
▫️Left ventricular systolic and diastolic function were worse in MIS-C compared to KD, and although systolic dysfunction in MIS-C patients recovered quickly within several days, diastolic dysfunction persisted.
- In addition to the left ventricle, both the right ventricle and left atrium - sometimes called the "forgotten chambers" of the heart - were significantly affected.

Role of Aspirin Explored in Primary Prevention of CVD in Systemic Rheumatic Diseases

▫️Low-dose Aspirin may be considered for the primary prevention of Cardiovascular disease (CVD) in patients with autoimmune systemic rheumatic diseases who are at particularly high risk because of their individual cardiovascular risk profile, according to authors of a new review article in the Journal Rheumatology who acknowledge the controversial nature of the issue, because while significant cardiovascular benefit from aspirin for secondary prevention is well established, it has not been for primary prevention.

Observational Study Again Suggests Lasting COVID-19 Impact on Heart

▫️A new study using Cardiac Magnetic Resonance (CMR) imaging to examine the effects of novel Coronavirus infection on the heart showed signs suggestive of Myocarditis in 4 out of 26 competitive athletes who recovered from asymptomatic or mild cases of COVID-19.
▫️In the Research Letter published September 11 in
JAMA Cardiology, Two of the 4 patients showing signs of Myocarditis in this series had no symptoms of COVID-19 but tested positive on routine testing.
- An additional 12 student athletes (46%) showed Late Gadolinium Enhancement (LGE), of whom 8 (30.8%) had LGE without T2 elevation, suggestive of prior myocardial injury.

Prone ECGs in COVID19 ICU Patients
Many hospitals are obtaining 12-lead ECGs in prone-ventilated patients by placing leads on the patient’s back, posterior to lead placement in the traditional supine position. The hearts of prone patients fall forward (ventrally), abut the anterior chest wall, and move caudally, away from posteriorly placed leads. The more distant position of the heart from the leads, coupled with greater impedance from soft tissue (including disease lung in COVID19 patients) and bone interposed between myocardium and electrodes causes diminished QRS amplitude in leads V1-V5 and prominent Q waves in leads V1-V3, which may be misinterpreted as an anteroseptal infarct, unless the positioning is properly annotated.

افزایش احتمال عوارض قلبی با مصرف همزمان آزیترومایسین با داروهای طولانی کننده QT قلبی .
این مقاله روز گذشته در جاما منتشر شده است .
در این بررسی مصرف تنهایی آزیترومایسین در ایجاد عوارض قلبی نقشی نداشته است .
مصرف همزمان آزیترومایسین با داروهای طولانی کننده QT نظیر مهار کننده های ACE مثل کاپتوپریل، بلاک کننده های بتای ادرنرژیک مثل آتنولول یا داروهایی ضد مالاریا مثل کلروکین و هیدروکسی کلروکین، برخی از داروهای ضد افسردگی و اپیوئیدها نظیر تریاک می تواند احتمال بروز عوارض قلبی نظیر طپش قلب، غش و حتی حمله قلبی را تا ۴۰ درصد افزایش دهد .
نتایج بر اساس بررسی پرونده چهار میلیون بیمار به دست آمده است.
